Patient Process
The Pure Care Cannabis Clinic assists our patients throughout the ENTIRE medical marijuana recommendation process AND will directly assist you with submission of your licensing documents. We streamline the process of applying by including the required forms. Your application will be FULLY COMPLETED when you leave our offices.
Medical Records or Prescription Medications
Our staff will assist you with ALL of the required forms and paperwork that need to be submitted to complete your application.
Please bring your:
(1) Arkansas driver’s license or state ID card;
(2) medical records supporting your qualifying condition OR
(3) a list of your prescription medications (or you may also just bring the prescription bottles instead of a list).
Consultation
You will meet one-on-one with one of our board licensed medical marijuana physicians to review and discuss your medical history.
Application
We directly assist you with submission of all the required documents to the State of Arkansas Department of Health website from our comfortable workstations. Your application will be 100% complete and submitted when you leave our offices.

We accept cash, debit card or credit card for the physician’s visit.
You will need a credit card, debit card or prepaid gift card the state license fee of $50.00 that is paid directly to the state for your one-year license.
