We provide your medical marijuana patient recommendation from our physician AND our staff assists you with the completion and uploading ALL of your documents for your license. Your application will be FULLY COMPLETE when you leave our offices.
Please bring your:
(1) Arkansas driver’s license or state ID card;
(2) medical records supporting your qualifying condition OR
(3) a list of prescription medications from your pharmacy or doctor for the last 6 months.
If you do not have your medical records or list of prescription medications, please give us a call at 501-291-0606 to discuss.

Military Veteran – MMJ Recommendation
Special offer for our military veterans with valid military ID or other military service documentation. We provide your medical marijuana patient recommendation from our physician AND our staff assists you with the completion and uploading ALL of your documents for your license. You will need a credit card, debit card or prepaid gift card the state license fee of $50.00 that is paid directly to the state for your one-year license.
